A benzodiazepine (pronounced /ˌbɛnzɵdaɪˈæzɨpiːn/, sometimes colloquially referred to as a "benzo", and often abbreviated in the literature as a "BZD") is a psychoactive drug whose core chemical structure is the fusion of a benzene ring and a diazepine ring. The first benzodiazepine, chlordiazepoxide (Librium), was discovered accidentally by Leo Sternbach in 1955, and made available in 1960 by Hoffmann–La Roche, which has also marketed diazepam (Valium) since 1963.

Benzodiazepines enhance the effect of the neurotransmitter gamma-aminobutyric acid, which results in sedative, hypnotic (sleep-inducing), anxiolytic (anti-anxiety), anticonvulsant, muscle relaxant and amnesic action. These properties make benzodiazepines useful in treating anxiety, insomnia, agitation, seizures, muscle spasms, alcohol withdrawal and as a premedication for medical or dental procedures. Benzodiazepines are categorized as either short-, intermediate- or long-acting. Short- and intermediate-acting benzodiazepines are preferred for the treatment of insomnia; longer-acting benzodiazepines are recommended for the treatment of anxiety.

In general, benzodiazepines are safe and effective in the short term, although cognitive impairments and paradoxical effects such as aggression or behavioral disinhibition occasionally occur. Long-term use is controversial due to concerns about adverse psychological and physical effects, increased questioning of effectiveness and because benzodiazepines are prone to cause tolerance, physical dependence and upon cessation of use, a withdrawal syndrome. In general, withdrawal from benzodiazepines leads to improved physical and mental health. The elderly are at an increased risk of suffering from both short- and long-term adverse effects.

There is controversy concerning the safety of benzodiazepines in pregnancy. While they are not major teratogens, uncertainty remains as to whether they cause cleft palate in a small number of babies and whether neurobehavioural effects occur as a result of prenatal exposure; they are known to cause withdrawal symptoms in the newborn. Benzodiazepines can be taken in overdoses and can cause dangerous deep unconsciousness. However, they are much less toxic than their predecessors, the barbiturates, and death rarely results when a benzodiazepine is the only drug taken. When combined with other central nervous system depressants such as alcohol and opiates, the potential for toxicity increases. Benzodiazepines are commonly misused and taken in combination with other drugs of abuse.

Are there any Benzodiazepines that you CAN take while pregnant?
Q. My sister had bad postpartum with her first child. She is expecting again, but this time around, she is afraid of getting really bad panic attacks again. I know you can take Zoloft while pregnant but that doesn't quite work while having a panic attack episode. Are there any safe Benzodiazepines for pregnancy?

A. Yes. Your sister should speak to her obstetrician. If she doesn't have one who is comfortable having this discussion with her, she should ask to be referred to an MFM (maternal fetal medicine specialist).

How would tolerance and overdose be different between opioids and benzodiazepines?
Q. How would tolerance and overdose be different between opioids and benzodiazepines?

A. Tolerance to a medication means that after someone takes a certain dose of a medication for a period of time, the dose is no longer effective. They need to take a higher dose in order to get the same effects as before. A common one is Morphine. There are people who can take HUGE amounts of Morphine, doses that woudl kill you or I, and for them it just barely touches the pain. Overdose is where they are still getting the effects of the drug and the drug overwhelms their system. Like with Heath Ledger. He took too much of too many different drugs. Their interactions combined and overwhelmed his system and stopped his breathing. Hope that helps!

What is the best way to get a prescription for benzodiazepines long term?
Q. I have social anxiety disorder, and these are the only medicines I've found that work. Are there any types of doctors that are more likely to go about prescribing them? How do you go about this? Please don't tell me to take anti-depressants because they don't work well enough for me.

A. usually if you are going to be taking benzo's it is likely to be complemented by anti-depressants. just talk to your doctor, anti depressants are compeletly different from benzo's, and if you tell a doctor you just like benzo's, which are very often abused, it might be confusing for him. just talk to a doctor about the symptoms you have and what types of drugs that you have used that have helped you in the past and they will look into it and if it indeed what you need then they will prescribe it. so just talk to a doctor, im assuming you are interested in something like xanax? those medicines are often most beneficial when coupled with other drugs that help out your brain chemistry. im not gonna tell you to take anti depressants, but if a… [cont.]
